Please bring a photo ID, your insurance card, a list of current medications (including dosages), and any relevant medical records or pathology reports from your referring physician. If you have a companion, they're welcome to join you.
We accept most major insurance plans. Our financial navigation team will verify your benefits before your appointment and help you understand any out-of-pocket costs. We also offer assistance programs for eligible patients.
